Important note for customers with Medicare benefits:

If you enroll in the Medicare-Medicaid Alignment Initiative (MMAI), your health plan will cover all of the basic plan benefits listed above.
If you enroll in HealthChoice Illinois, your health plan will only cover these Long Term Services and Supports:
- Your long term care services if you live in a nursing home or supportive living facility
- Your home-based waiver services (such as the Aging or Physically Disabled waivers)
- Non-emergency transportation and some behavioral health services
